There are many different types of breast augmentation surgeries and many different ways that implants can be placed. The different types of procedures used to place breast implants are: trans-axillary, peri-aeriolar, trans-umbilical, or inframammary (sub-pectoral). In our video, the implant is placed using the inframammary fold (sub-pectoral) technique. The sub-pectoral procedure involves an incision in the fold underneath the breast and tends to allow the surgeon more direct control of implant positioning. About 70 to 80 percent of breast augmentations are performed using an inframammary fold procedure. The incision placement allows for minimal effects on the milk-producing areas of the breast. The same incision may also be used if complications arise or additional surgeries are needed. Inframammary fold augmentation works well for older patients or mothers, since age and pregnancy allows for a natural amount of sag in the breast. This natural sag allows for the surgical incision to be well hidden for most women. Young women, thin women, or women who have not had children yet and have no crease under their breasts may have a more visible scar with this procedure.

If there are multiple areas that a patient wants to address cosmetically, depending on the areas, multiple procedures can be performed during the same surgery (often referred to as a "Mommy Makeover"). This is the case in this video.

Breast augmentation - is a surgical technique to increase the size of the breasts. Any woman that desires a larger breast may be a candidate for breast augmentation if she has realistic expectations. There are other techniques to increase the size of the breasts, but the most trusted technique is placement of an implant surgically. There are multiple options that are discussed and decided during a breast augmentation consultation. It is not uncommon for a breast augmentation procedure to include both breast implants and a breast lift in order for the patient to achieve maximum cosmetic satisfaction.

Breast Lift - Sagging or ptotic breasts occur with age, pregnancy, and weight fluctuations, but can be improved with a breast lift, or mastopexy. In the case of mild ptosis or deflation hypomastia, a breast implant alone may correct the deformity. Moderate ptosis can be corrected by excision of skin in a doughnut fashion around the areola. More severe sag will require a vertical scar or anchor shaped scar below the nipple to tighten the loose breast around the elevated nipple.

Belt Lipectomy - All layers of the abdominal wall (skin, fat, fascia, and muscle) are affected by changes occurring with age, pregnancy, and weight change. A circumferential body lift, or belt lipectomy, is indicated in patients with excess skin and fat of the abdomen, flanks, and back with laxity and descent of the buttock and hips, often seen after massive weight loss.

This particular procedure is just one of the many different combinations of surgeries that can be done in what is considered a "Mommy Makeover". The procedure in this video took between 3 to 4 hours, however, we sped up the video so that viewers could get the full experience without having to sit through a 3 to 4 hour video. Belt Lipectomy/Lower Body Lifts - The Park Clinic for Plastic Surgery

Our Cosmetic Coordinator, Darlene, sits down with Dr. Chris Park and discusses lower body lift procedures. A lower body lift is a surgical procedure designed to remove excess skin from the outer thighs and buttocks. It is a procedure that commonly is performed in patients who have lost a large amount of weight and have loose sagging skin. This procedure can dramatically tighten the loose skin on the outer thighs and buttocks. This procedure is also commonly called a "belt lipectomy".



What is a lipectomy? A lipectomy is a plastic surgery method that removes fat and skin. You can get a lipectomy nearly anywhere on your body, including common trouble spots like the back of your arms, inner thighs, hips, belly, and more. If you get a tummy tuck, that is technically a lipectomy of the tummy. A belt lipectomy is a removal of said tissue that resides around the belt line.

Labiaplasty & Other Labiaplasty Options at The Park Clinic

Darlene, our Cosmetic Coordinator, sits down with Dr. Christopher Park to discuss vaginal rejuvenation and labiaplasty options at The Park Clinic in Mobile, Alabama. Labiaplasty is one of the fastest growing plastic surgery procedures in the United States, and it is one that Dr. Park and Dr. Rebowe do a lot of, yet there is not a lot of educational content on the procedure for patients to research.



The term vaginal rejuvenation covers several different procedures. It is sometimes called female genital plastic surgery, female genital rejuvenation surgery, female genital cosmetic surgery, vulvovaginal plastic surgery and designer vagina surgery, among other terms. Among the procedures that comprise female genital plastic surgery are labiaplasty, clitoral hood reduction, labia majoraplasty, monsplasty and vaginoplasty.



The term labiaplasty refers to a procedure that reduces the length of the labia minora. It is the most commonly performed labiaplasty procedure and it can relieve symptoms women experience from twisting and tugging of the labia. Women opt for surgery for a variety of reasons, including pain from twisting and tugging of the labia when riding a bike or during intercourse, itching, irritation and self-consciousness. The goal of the procedure is to reduce the labia minora so that they don't hang below the hair-bearing labia majora. A labiaplasty may be performed to reduce asymmetry when one is longer than the other, or, more commonly, to reduce the length of both labia so that the labia no longer twist, tug or fall out of a bathing suit. The most common type of labiaplasty is the trim procedure, in which the extra tissue is removed and sewn up directly. Next in popularity is the wedge procedure, which maintains a natural border after a pie-shaped piece of tissue has been removed. Extra folds of the clitoral hood can also be reduced at the same time. Closure is usually done with absorbable sutures. Most patients take a week off from work, during which they can reduce swelling and pain by icing with a cold pack sandwiched between the patient's underpants and an elastic garment, like Spanx. This can be done "twenty minutes on, twenty minutes off." The patient can also lie with her bottom elevated to reduce swelling. Patients can resume wearing tampons or having intercourse after four to six weeks. Trim labiaplasty generally allows for a quicker recovery.


Given both the popularity of the procedure and the privacy and delicacy of the area being treated, many women may not be comfortable talking about the procedure. Breast Reduction Clinic - The Park Clinic for Plastic Surgery

Breast Reductions are the most common procedures we perform at The Park Clinic. We have a very unique approach to our evaluation of patients. We have an entire process dedicated to our Breast reduction Clinic

When you make an appointment for a Breast Reduction consultation you will be making an appointment to see me first. I will become your personal guide through all curves of the road ahead of you-consultation, examination, insurance approval, pre-op, surgery, recovery, and post op appointments. Over the years I have become an expert at breast reduction consultations, and I take pride and joy in discussing this surgery, examining you, and helping you to feel comfortable about the possibility of a procedure that will change your life!
At your first consultation appointment we will discuss the surgical procedure in great detail. Have all of your questions ready because I will be ready to answer them! Photos will be taken of you for insurance purposes only. I will examine you and take many measurements which are necessary to submit to your insurance company in order to obtain insurance approval. After the physical examination I will discuss in more detail about your current size, what size you would like to ideally be, and how much I feel can be removed safely while leaving you cosmetically beautiful. Certain plans allow us to submit your measurements online and you may know within minutes whether you have been approved allowing us to schedule your surgery and pre-op appointment with one of our surgeons before you leave your appointment! Some insurance companies require us to mail a letter with your measurements and pictures. If we have to mail a letter it can take anywhere from 4-6 weeks to get an answer from your insurance company. Once you have heard that you have been approved you can then call our office to schedule your surgery and pre-op appointment. We do require a current mammogram within 1 year as well as a BMI less than 40 and preferably less than 35.
Your pre-op appointment will be with one of our renowned surgeons, Dr. Christopher Park or Dr. Ryan Rebowe. During this appointment you will be examined once again by the surgeon performing your surgery, and they will welcome any questions you have before your big day. Consents for surgery will also be signed by you this day and you will receive any medications you will need post-op. At this appointment we will make sure you are ready for your surgical procedure.
Your surgery can be schedule at any of the facilities where we operate. Usually the facility is dictated by the day of surgery you desire and with whom your surgery is scheduled. The actual surgery usually take around 2 ½-3 hours. All of the facilities offer impeccable care and we will be there to help comfort and relieve your anxiety. Breast reduction surgery is an outpatient procedure which means you can be discharged with your loved ones the same day. Spending the night in the hospital is a possibility if one of us feels it is necessary though. You will be sent home from the surgical facility in a bra or breast binder, so you do not need to purchase many bras before your surgery. If you want to have an extra breast garment at home for after your surgery we recommend a soft, non-wired bra and one that fastens in the front. You also may have a drain placed during surgery to remove any excess fluid during your post op time. It will be important for you to record the drainage output and bring that record with you for your 1 week post-op appointment. The medical facility will give you a drain log and post op instructions. Feel free to call our office if you have any questions though. We are always happy to answer your questions. I will see you back at our office approximately 1 week post-op. I will remove your steri strips and your drain if you have one at this time. Don’t worry, it doesn’t hurt to remove your drain!

Many patients also ask about adding other cosmetic procedures at the same time as their breast reduction surgery. It is very common for us to perform other surgical procedures at the time of a breast reduction. Some of the other common procedures include liposuction, abdominoplasty, and body contouring. Rhinoplasty Overview - The Park Clinic for Plastic Surgery

Necklift vs. Facelift - The Park Clinic for Plastic Surgery

Neck lift (also called platysmaplasty) is a cosmetic plastic surgery procedure that removes excess skin and fat from the neck to address age-related wrinkling and creasing in this area, and provide a smoother, slimmer profile.

Getting a neck lift starts with a candid discussion with one of our surgeons. You and our expert will talk about your medical history and your general health, skin condition, scarring tendency and other factors in the context of what you hope to achieve by getting a neck lift.

The procedure takes several hours, during which the surgeon makes small incisions around and behind your ears, and a very small one under the chin. DIEP Flap Breast Cancer Reconstruction - The Park Clinic for Plastic Surgery

A DIEP flap is a type of breast reconstruction in which blood vessels called deep inferior epigastric perforators (DIEP), as well as the skin and fat connected to them, are removed from the lower abdomen and transferred to the chest to reconstruct a breast after mastectomy without the sacrifice of the patient's abdominal wall.

Dr. Rebowe gives a quick overview of why the procedure is optimal for the breast cancer victim and what makes the outcome better than a few other options.

Dr. Park and Dr. Rebowe are the only physicians that offer the DIEP flap procedure in the Mobile area.

Breast reconstruction flaps create a breast by moving tissue from another part of one’s own body (autologous). The advantages are increased durability to radiation, more natural shape and feel, and the ability to add tissue where tissue is missing. In suitable patients, Dr. Park prefers flap reconstruction. Blood supply is critical for flap reconstruction and is impaired in patients who are smokers, diabetic, obese, or suffer from vascular disease, collagen vascular disease, and autoimmune diseases. The most common source for breast reconstruction flaps is the abdomen. The primary advantage of using abdominal tissue is that it provides the most cosmetic and natural breast reconstruction in women with the best skin and texture match to the breast. In addition, a modified version of an abdominoplasty or tummy tuck is performed at the same time, but this should not drive the decision. The potential complications of implants are also usually avoided.

The skin and fat of the lower abdominal wall, normally discarded in a tummy tuck operation, are elevated and moved to the chest using either the rectus muscle as a pedicle or hinge with blood supply (Pedicled Transverse Rectus Abdominis Myocutaneous /TRAM flap), disconnecting the tissues blood supply to the muscle and skin and moving it to the chest (Free Transverse Rectus Abdominis Myocutaneous / Free TRAM flap), a free TRAM with limited muscle (Muscle sparing free TRAM), or moving the tissue based only on a blood vessel, skin, and fat (Deep Inferior Epigastric Perforator / DIEP flap). The last 3 options mentioned are much more complicated and time intensive, requiring division of vessels and reconnection to blood vessels in the chest or armpit using a microscope, similar to a transplant. Once completed, the amount of blood flow is greater than the pedicled TRAM, but there is a risk of thrombosis (clotting) of the anastamoses, creating the need for urgent reoperation and the risk of complete loss of the flap. These procedures should only be performed by those experienced in their use.

Tissue Matrix In Breast Cancer Reconstruction Patients

Have you ever heard of a "tissue matrix" in breast cancer reconstruction? Dr. Christopher Park and Dr. Ryan Rebowe of The Park Clinic for Plastic Surgery use tissue matrices often in the breast cancer reconstruction cases they perform.

"Tissue matrix" is a generic term referring to large sheets of collagen matrix used in breast cancer reconstruction to help add soft tissue support to implant based breast reconstruction. The native skin will incorporate this matrix and it will become part of the mastectomy skin flaps, decrease the rate of capsular contracture, and allow for more precise placement of the tissue expander or implant used in your reconstruction. In some cases a tissue matrix allows us to place the implant above the muscle.

In the video above you see a smooth round moderate plus silicone implant used for a "direct to implant" reconstruction following a nipple sparing mastectomy. Using an implant to rebuild the breast requires less surgery than flap reconstruction, since it only involves the chest area (and not a tissue donor site). Still, it may require more than one procedure. It also may require additional surgery in the future, as implants can wear out and develop other issues, such as tightness of scar tissue around the implant. Implants may be a good option for thin women with small breasts because these women often don't have enough extra tissue on their bellies, backs, thighs, or buttocks to form a good breast shape as required by flap reconstruction. Implant reconstruction is also a good option if:

-- You’d like to avoid incisions in other parts of the body (donor sites) or sacrificing the muscle structure in donor sites. Also keep in mind that the newest flap procedures preserve muscle and may be worth exploring if you’re concerned about this.
-- You do not need radiation therapy. There is a high chance of developing problems in an implant reconstruction after radiation.
-- You can’t or don’t want to endure a lengthier flap reconstruction operation.
-- You are willing to surgically alter your healthy breast to achieve symmetry or balance. It is not always easy to match an implant, which has a fixed shape, to the remaining natural breast.

An abdominoplasty (tummy tuck) is a major surgery that removes excess skin and fat from the abdomen. The surgery also tightens muscles. It can be appropriate for women who have had many pregnancies or anyone who has lot a lot of weight. An abdominoplasty is a cosmetic operation not covered by insurance aimed at re-contouring the abdominal wall, but muscle tightening also may improve low back discomfort and strength. The operation can be safely performed in an operating room facility with general anesthesia, deep sedation, or epidural block. Incisions are designed to place scars under a preferred underwear or bathing suit pattern. After elevating the abdominal wall and re-draping, excess skin and fat are removed and the abdominal muscles are tightened if necessary to improve the waist line. Often the vertical abdominal muscles are separated, a “diastasis rectus,” and the hourglass narrowing above the hips are lost, both of which can be improved with internal sutures. The existing navel (umbilicus) is preserved and transposed. The procedure takes approximately 2 to 3 hours.

Who makes the best candidate for a tummy tuck? A tummy tuck is suitable for both men and women who are in good general health overall and are at a stable weight. It is best to be a non-smoker. A tummy tuck should not be confused with a liposuction (the cosmetic surgery used to remove fat deposits), although your surgeon may choose to perform liposuction as part of a tummy tuck. Women who have muscles and skin stretched after several pregnancies may find the procedure useful to tighten those muscles and reduce that skin. A tummy tuck is also an alternative for men or women who were obese at one point in their lives, and still have excessive fat deposits or loose skin in the abdominal area.

Down time with a tummy tuck: Overnight observation, drainage tubes, maintaining a flexed position, and a compression binder are routine. Pain can be controlled initially with prescription medication, but tapers rapidly several days after the procedure. There will be a general soreness of the muscles and the back for several weeks. Laying and standing straight can be resumed at 7 – 10 days as tolerated. Most individuals return to work 3 weeks later depending on requirements. Strenuous activity and lifting (20 + pounds) can slowly escalate between 6 weeks and 12 weeks. The downsides are the scar and common surgical risks: fluid collections, bleeding, infection, and healing difficulties.

Large, heavy breasts can cause a number of problems including neck pain, back pain, bra strap grooving, upper extremity nerve compression, rashes, or yeast infections, as well as considerable embarrassment, especially to younger women and teenagers. Women experiencing any of these problems may be candidates for a breast reduction. A breast reduction removes excess tissue and returns the descended nipple/areola to a more normal size and position, producing more comfortable, proportional, and aesthetic breasts. Excessively large breasts (macromastia) are often the result of increased sensitivity to estrogen and may occur due to genetics, weight gain, or endocrine disorders. Teenagers may develop macromastia with hormonal changes known as virginal hypertrophy and observation is recommended as it may improve or recur if a reduction is performed in teenagers, but if significantly traumatic to the psyche, a reduction mammoplasty can be helpful.

The biggest drawback to a breast reduction is scarring, although most patients are happy with the trade off. Most breasts will require an inferior pedicle wise pattern reduction which leaves scars that extend around the areola, down to the inframammary fold, and across the inframammary fold in the shape of an anchor. Certain smaller breast can be reduced without the transverse scar below the breast, using a vertical breast reduction technique either medially or superiorly based pedicle. This method often leaves some excess skin at the bottom of the breast that dissipates in time, but may require a minor excision later. Projection is often greater with this technique, but 6 – 12 weeks of settling is necessary before the final result is appreciated. The nipple is left attached to blood and nerve supply and rotated up before adjusting the skin envelope for closure. Sensation, nipple survival, and nipple function should remain normal. Patients with extremely large breasts (gigantomastia) may require a technique where the lower breast is removed and the nipple is transferred as a free graft, disrupting the sensation and functional ducts.

Breast augmentation — also known as augmentation mammoplasty — is surgery to increase breast size. It involves placing breast implants under breast tissue or chest muscles. For some women, breast augmentation is a way to feel more confident. For others, it's part of rebuilding the breast for various conditions.

There are many different types of breast implants available to patients:

Saline breast implants are filled with sterile salt water. Should the implant shell leak, a saline implant will collapse and the saline will be absorbed and naturally expelled by the body. Saline breast implants provide a uniform shape, firmness and feel, and are FDA-approved for augmentation in women age 18 or older.

Structured saline breast implants -- Structured implants are filled with sterile salt water, and contain an inner structure which aims to make the implant feel more natural.

Silicone breast implants -- Silicone breast implants are filled with silicone gel. The gel feels a bit more like natural breast tissue. If the implant leaks, the gel may remain within the implant shell, or may escape into the breast implant pocket. A leaking implant filled with silicone gel will not collapse. If you choose silicone implants, you may need to visit your plastic surgeon regularly to make sure the implants are functioning properly. An ultrasound or MRI screening can assess the condition of breast implants. Silicone breast implants are FDA-approved for augmentation in women age 22 or older.

Gummy bear breast implants -- Form-stable implants are sometimes referred to as gummy bear breast implants because they maintain their shape even when the implant shell is broken. The consistency of the silicone gel inside the implant is thicker than traditional silicone gel implants. These implants are also firmer than traditional implants. Shaped gummy bear breast implants have more projection at the bottom and are tapered towards the top. If a shaped implant rotates, it may lead to an unusual appearance of the breast that requires a separate procedure to correct. Placement of gummy bear implants requires a slightly longer incision in the skin.

Round breast implants -- Round breast implants have a tendency to make breasts appear fuller than form-stable implants. Higher profile options can achieve even more projection. Because round implants are the same shape all over, there is less concern about them rotating out of place.

Smooth breast implants -- Smooth breast implants are the softest feeling. They can move with the breast implant pocket, which may give more natural movement. Smooth implants may have some palpable or visible rippling under the skin.

Textured breast implants -- Textured breast implants develop scar tissue to stick to the implant, making them less likely to move around inside of the breast and become repositioned. Texturing offers some advantage in diminishing the risk of a tight scar capsule.

Returning to our discussion on the case in the video above, below is more information on the individual procedures combined in this Mommy Makeover.

A breast lift, also known as mastopexy, raises the breasts by removing excess skin and tightening the surrounding tissue to reshape and support the new breast contour.

New statistics from the American Society of Plastic Surgeons show breast lifts have grown 70% since 2000, outpacing breast implants 2-to-1.

Sometimes the areola becomes enlarged over time, and a breast lift will reduce this as well.

Liposuction is a surgical procedure that uses a suction technique to remove fat from specific areas of the body, such as the abdomen, hips, thighs, buttocks, arms or neck. Liposuction also shapes (contours) these areas. Other names for liposuction include lipoplasty and body contouring of the abdomen, buttocks, flanks, and limbs.

Scar Revision - The Park Clinic for Plastic Surgery

The degree of improvement that can be achieved with scar revision will depend on the severity of your scarring, and the type, size and location of the scar. In some cases, a single technique may provide significant improvement. However, your plastic surgeon may recommend a combination of scar revision techniques to achieve the best results.

Topical treatments, such as gels, tapes or external compression, can help in wound closure and healing, or to reduce the ability of skin to produce irregular pigment. These products may be used to treat existing surface scars and discoloration, and to aid in the healing of scar revision procedures.

Injectable treatments may also be used. Dermal filler can be used to fill depressed or concave scars. Depending on the injectable substance used and your particular scar conditions, results may last from three months to several years. Therapy must be repeated to maintain results. Another form of injection therapy uses steroidal-based compounds to reduce collagen formation and can alter the appearance, size and texture of raised scar tissue.

Surface treatments are most often used for cosmetic improvement of scars. These methods can soften surface irregularities and reduce uneven pigmentation. Surface treatments are a controlled means of either mechanically removing the top layers of skin or changing the nature of tissue.

Cyst Removal - The Park Clinic for Plastic Surgery

Dr. Ryan Rebowe of The Park Clinic for Plastic Surgery performs a quick cyst removal. This is a quick and easy procedure that we do in our office. The procedure can be performed under general or local anesthesia depending on the size of the cyst. This procedure is often covered by insurance.

Cysts are usually benign slow growing “sacks” of tissue that can occur anywhere on the body, though often found on the face and scalp. Cysts can drain, rupture, grow larger, become infected, and deform surrounding tissue. Careful removal is recommended. Common benign bumps include lipoma, epidermal inclusion cyst, sebaceous cyst, infundibular cyst, pilar cyst, dermoid cyst, pilomatrixoma, pilomatricoma, osteoid osteoma. Removal provides an opportunity to determine a diagnosis. Imaging studies such as CT scan, MRI or Ultrasound may be helpful though rarely necessary.

How Much Lip Filler Is In One Syringe? - The Park Clinic for Plastic Surgery

Dr. Park and Dr. Rebowe get asked often about how much filler is in one syringe of filler. Most often this is asked because the patient is worried about looking "over-filled".

Ruptured Implant In An Older Generation Implant - The Park Clinic

Dr. Rebowe has discussed ruptured implants recently in another video. In this video, while doing an explant on a patient, he stops to show viewers what an older generation implant looks like when ruptured in the body. This particular implant is about 30 years old.

Last week we posted about gummy bear implants and cohesive gels. This case illustrates why the newer generation implants were developed. Older implants had a very thin gel that tended to leak through their thin silicone shells, and the same silicone shells had a tendency to rupture even with minor trauma. Today’s implants are much stronger and typically do not rupture except for extreme circumstances. When they do, rupture is typically contained within the scar capsule around the implant due to the cohesive or “gummy” nature of the gel. Thinner gels of older implants do have a tendency to leak into tissues if left long enough, however this patient thankfully had an intra-capsular rupture. Both implants and the scar capsule in this case were removed in order to ensure no silicone remained.

The implant is a silicone shell filled with sterile saline (salt water) or silicone gel. The silicone gel filled implants have been again used for cosmetic use after FDA clearance in 2006 after the highly publicized controversy regarding the long term health consequences of silicone gel implants. The findings of all research studies have not demonstrated any consistent relationship between silicone breast implants and systemic illness. The new generation of implants is less prone to the slow silicone leak (bleed) that was felt to be responsible for most of the local problems with silicone implants. Gel implants are more soft and natural, especially when there is less overlying breast tissue, but they do require larger incisions than their saline counterparts since they are prefilled.

Free Tissue Transfer of Lower Extremity - The Park Clinic

In this video Dr. Rebowe walks viewers through how he closes a leg wound that resulted from a bone fracture using a free flap technique by taking tissue from the patient’s thigh and attaching it to the patient’s ankle wound. This is the area of plastic surgery that is not often talked about or glorified but is equally as gratifying because of the skill required to perform free tissue transfers.

Tissue transfers are a form of "microsurgery" performed by plastic surgeons. Reconstructive microsurgery is a surgical discipline in which specialized operating microscopes and precision instrumentation are used to repair intricate structures such as blood vessels and nerves less than a few millimeters in diameter. Capsulectomy Procedure - The Park Clinic for Plastic Surgery

A capsulectomy refers to the surgical removal of the capsule (made of scar tissue) that forms naturally around breast implants. This surgery is typically used to treat a complication of breast augmentation, removing implant ruptures, or breast reconstruction surgery called capsule contracture. We perform many of these at The Park Clinic for Plastic Surgery in Mobile, Alabama. Most of the time when we perform these surgeries they are due to implant ruptures.

All types of breast implants have an outer, silicone shell that can potentially develop a tear or hole called a rupture. A saline implant also has a valve that can fail, causing a leak. The chance that your implant will rupture or leak increases over time. Implant rupture can happen because of normal aging of the implant, trauma caused by a car accident, a needle insertion during a biopsy, or other factors.

As plastic surgeons, we get to take part in some pretty cool surgeries that overlap with other specialties.
